Your hip replacement, week by week
Enter your surgery date on the home page and this page shows the stage you are in today, the rules that still apply, and what comes next.
This guide is for a routine first-time hip replacement for arthritis. If you had a revision, both hips at once, or a fracture, your recovery will be slower and your surgeon's timeline comes first.
Your precautions
Until your precaution end date — 6 weeks after surgery unless your surgeon tells you longerYour new hip is held in place by the muscles and tissues around it, and they need time to heal before they can protect it fully. Until then, three movements can push the ball out of the socket. Avoid them in everything you do:
- Do not bend the hip past 90°. Your knee never comes higher than your hip. No low chairs, no leaning forward to reach your feet, no pulling the knee to your chest.
- Do not cross your legs at the knees or the ankles, sitting or lying, and do not let the operated leg roll inwards.
- Do not twist your body or your legs. To turn around, take small steps with your feet; never pivot on the operated leg. Keep the toes pointing forward or slightly out.
And do:
- Sleep with a pillow between your knees whenever you lie on your side, and keep it there when lying on your back for the first weeks. A long body pillow is easiest.
- Sit high. Use a raised chair or a firm foam cushion, and a raised toilet seat. The seat should be about 5 cm higher than your knee when you stand next to it.
- Lift the operated leg with a strap or leg lifter to get it in and out of bed; do not hook the other leg under it.
- Use long-handled aids — reacher, shoehorn, sock aid — for anything below knee level, including picking things up from the floor.
- Shower on a shower chair or a bath board. Never sit on the bottom of the bath.
- In the car, seat all the way back and a cushion on the seat so the hip stays above 90°.

If your surgeon gave you a different date
The site assumes 6 weeks. If your surgeon told you to keep the precautions for longer, enter that date here and every page will use it instead.
When precautions end
Book an appointment with your surgeon for the week your precautions end. The surgeon checks that you are ready and clears you to come out of the precautions; bring any questions that have come up along the way.
After that visit you may bend past 90°, cross your legs and sit low again. Do it gradually: a little further each day over two to three weeks rather than all at once. The raised toilet seat and chair cushion can go.
Putting weight through the leg
You can put your full weight through the new hip from the first day, using the walker for balance. The only exception is if your surgeon has told you it is safer to delay — for example because of weak bone. In that case your surgeon will tell you how much weight the leg can take and for how long, and that instruction replaces everything on this page about walking and weight. If you are not sure whether this applies to you, ask at your wound check.
Walking aids
Walker first, then crutches or a cane in the hand opposite the operated hip, then nothing. For most people the walker gives way to a cane at about 2–3 weeks, and the cane goes somewhere between 6 and 9 weeks. Your physiotherapist moves you on when your balance and your walking pattern are ready, not on a fixed day. Keep using the aid until you can walk without a limp; a limp now becomes a habit later.
The five stages
Weeks 0–3 · Protect, move, walk
Focus: pain control, sleep, getting to the toilet on your own, getting the hip moving, and standing and walking a little more each day.
Before your wound check at 2 weeks you should be:
- walking to the toilet and using it yourself. Stop using a bedpan or commode as soon as you can stand with the walker — usually within the first few days. Staying on a bedpan slows everything else down.
- getting your leg in and out of bed on your own (a strap or belt around the foot helps at first)
- getting on and off the raised chair, the bed and the raised toilet without help
By the end of week 3 you should be able to:
- walk around the house with your walker or crutches
- go up and down stairs safely with your aid: up with the good leg first, down with the operated leg first
Exercises: ankle pumps every hour; hip exercises 3–16. Walk several short walks a day rather than one long one.
Washing: you may shower from week 2, after the wound check. No bath, pool or sea until week 4, once the wound is fully healed.
Appointments: wound check at 2 weeks. At this visit the dressing comes off and the wound is examined. By now it should be closed and dry with no leaking. If the wound was closed with staples or stitches that need removing, they are taken out at this visit; this takes a few minutes and does not need anaesthetic. If your wound is still leaking at any point after two weeks, do not wait for the next appointment — contact the clinic and arrange to be seen as soon as possible.
Weeks 3–6 · Range and control
Focus: the hip should hurt less and move more easily. Work on flexibility, strength and controlling the leg.
By the end of week 6 you should be able to:
- bend the hip to 90°, move the leg out to the side, and straighten it fully
- walk several hundred metres with a cane in the opposite hand
- pedal an upright bike with the seat raised, at the clinic or the gym
Exercises: add hip exercises 17–24. Exercise 19 waits until week 6.
Driving: left hip and an automatic car — once you are comfortable in the driver's seat and off strong painkillers. Right hip — not before 6 weeks. Check with your surgeon either way.
Appointments: surgeon review at 6 weeks. This and the 2-week wound check are the two visits that matter most; do not miss them.
Weeks 6–9 · Back to everyday life
Precautions end at 6 weeksFocus: returning to normal activities, and harder exercises agreed with your physiotherapist.
By the end of week 9 you should be able to:
- walk longer distances with a cane without limping
- stand up from and sit down on your raised chair without using your hands
- balance briefly on the operated leg
- climb stairs one foot per step with the rail or a cane
- manage a longer outing — a mall walk, a family visit — without paying for it the next day
Exercises: add 25–31 and the 14b progression.
Now allowed: gradual return to floor sitting and full prayer positions, side-sleeping without the pillow, low sofas, bending to put on your own socks and shoes.
Weeks 9–12 · Confidence and strength
Focus: feeling confident in daily life, and building strength and balance for the long term.
By the end of week 12 you should be able to:
- walk without a limp and without a cane
- stand up from a normal-height chair and toilet without hands
- climb stairs one foot per step with little or no rail
- join a gym or a walking group
Exercises: add 32–37 and the late progressions of 14, 22, 23, 28 and 31.
Appointments: surgeon review at 3 months.
3 months to 1 year · Fitness and sport
Focus: regular activity keeps the new joint healthy and makes it last longer. The operated leg takes up to a year or more to get all its strength back, so keep going.
How to keep progressing
- Strength: 8–15 repetitions; the muscle should feel tired by the last one. If you cannot do 8, make it easier. Once you can do 15 in three sessions in a row, make it harder: add 1–1.5 kg or a bigger range. Two days a week keeps strength, three builds it. Rest the day after, and never more than four strength days a week.
- Balance: 6–8 repetitions, 3 times a day, always with something to hold within reach.
- Flexibility: hold 30 s, 3–4 times, at least three days a week, when the muscles are warm.
Return to work: usually after the 3-month review. If your job is at a desk and you want to go back earlier, ask your surgeon at the 6-week review.
Appointments: surgeon review at 6 months and at 1 year. Book an appointment at any point in between if something concerns you.
Sport and activity
Choose activities that are low-impact and that you already know how to do. Learning a new sport on a new hip means more falls.

As a guide to how hard each activity works the joint: cycling loads the hip at about 1.4 times your body weight, brisk walking about 4.7 times, jogging about 6 times.
Normal at this stage
- Swelling in the thigh, leg and especially the foot and ankle on the operated side, worse at the end of the day. This is the most common question we get, and the answer is that it settles over weeks to months. Legs up when resting, ankle pumps, short walks rather than long ones, and avoid the heat. Move your toes often: this works the muscles in the foot and pushes the fluid back up the leg.
- Numbness beside the scar; it often stays.
- Clicking or a feeling of the joint "settling" in the first weeks.
- Poor sleep for up to 3 months.
- A flare of pain and swelling when you start a new exercise or go back to work. Ice, rest, and ease back.
your whole leg becomes swollen, warm or increasingly painful; the wound becomes red, opens or leaks; you develop a fever; the pain suddenly gets much worse; or the operated leg suddenly looks shorter or the foot turns outwards and you cannot move it — that may be a dislocation, go to the emergency department.
Go to the emergency department immediately for chest pain or sudden shortness of breath.

مفصل الورك الجديد، أسبوعًا بأسبوع

احتياطاتك
حتى انتهاء فترة الاحتياطات — ٦ أسابيع بعد العملية ما لم يحدد لك جرّاحك مدة أطولالمفصل الجديد تثبّته العضلات والأنسجة المحيطة به، وهذه تحتاج وقتًا لتلتئم قبل أن تحميه حماية كاملة. وإلى أن تلتئم، هناك ثلاث حركات قد تُخرج رأس المفصل من تجويفه. تجنّبها في كل ما تفعل:
- لا تثنِ الورك أكثر من ٩٠ درجة. ركبتك لا تعلو عن وركك أبدًا. لا كراسي منخفضة، ولا انحناء إلى الأمام للوصول إلى قدميك، ولا سحب الركبة نحو الصدر.
- لا تقاطع ساقيك عند الركبتين أو الكاحلين، جالسًا كنت أو مستلقيًا، ولا تدع الساق المعالَجة تنقلب إلى الداخل.
- لا تلتفّ بجسمك أو بساقيك. للاستدارة، خذ خطوات صغيرة بقدميك، ولا تدُر أبدًا على الساق المعالَجة. أبقِ أصابع القدم متجهة إلى الأمام أو إلى الخارج قليلًا.
وافعل الآتي:
- نَم ووسادة بين ركبتيك كلما استلقيت على جانبك، وأبقِها بينهما عند الاستلقاء على ظهرك في الأسابيع الأولى. الوسادة الطويلة أسهل.
- اجلس مرتفعًا. استخدم كرسيًا مرتفعًا أو وسادة إسفنجية متينة، ورافعًا لمقعد المرحاض. المقعد المناسب أعلى من ركبتك بنحو ٥ سم وأنت واقف بجانبه.
- ارفع الساق المعالَجة بحزام أو رافعة ساق لإدخالها إلى السرير وإخراجها منه، ولا تعلّق الساق الأخرى تحتها لرفعها.
- استخدم الأدوات ذات اليد الطويلة — الملقاط، لبّاسة الحذاء، أداة لبس الجوارب — لكل ما هو أسفل مستوى الركبة، بما في ذلك التقاط الأشياء من الأرض.
- استحم جالسًا على كرسي استحمام أو لوح حوض. لا تجلس أبدًا في قاع حوض الاستحمام.
- في السيارة، أرجع المقعد إلى الخلف تمامًا وضع وسادة على المقعد حتى يبقى الورك أعلى من ٩٠ درجة.

إذا حدد لك جرّاحك تاريخًا مختلفًا
يفترض الموقع ستة أسابيع. إذا طلب منك جرّاحك الاستمرار في الاحتياطات مدة أطول، أدخل ذلك التاريخ هنا وستعتمده كل الصفحات بدلًا من الافتراضي.
عند انتهاء الاحتياطات
احجز موعدًا مع جرّاحك في الأسبوع الذي تنتهي فيه احتياطاتك. يتأكد الجرّاح من جاهزيتك ويأذن لك بالخروج من الاحتياطات؛ وأحضر معك كل الأسئلة التي خطرت لك خلال هذه الفترة.
بعد تلك الزيارة يمكنك ثني الورك أكثر من ٩٠ درجة ومقاطعة ساقيك والجلوس منخفضًا من جديد. افعل ذلك تدريجيًا: زِد قليلًا كل يوم على مدى أسبوعين إلى ثلاثة، لا دفعة واحدة. ويمكنك الاستغناء عن رافع مقعد المرحاض ووسادة الكرسي.
تحميل الوزن على الساق
يمكنك تحميل وزنك كاملًا على الورك الجديد من اليوم الأول، مستعينًا بالمشّاية للتوازن. الاستثناء الوحيد إذا أخبرك جرّاحك أن التأخير أكثر أمانًا — بسبب ضعف العظم مثلًا. في هذه الحالة سيحدد لك جرّاحك مقدار الوزن الذي تتحمله الساق ولكم من الوقت، وتعليماته تلك تحلّ محل كل ما في هذه الصفحة عن المشي والوزن. إن لم تكن متأكدًا هل ينطبق هذا عليك، اسأل في موعد فحص الجرح.
أدوات المشي
المشّاية أولًا، ثم العكازان أو العصا في اليد المقابلة للورك المعالَج، ثم لا شيء. عند معظم الناس تحلّ العصا محل المشّاية في الأسبوع الثاني أو الثالث تقريبًا، ويُستغنى عن العصا بين الأسبوع السادس والتاسع. أخصائي العلاج الطبيعي هو من ينقلك من مرحلة إلى أخرى عندما يصبح توازنك ونمط مشيك جاهزين، لا في يوم محدد. استمر في استخدام الأداة حتى تمشي دون عرج؛ فالعرج الآن يصبح عادة لاحقًا.
المراحل الخمس
الأسابيع ٠–٣ · حماية، حركة، مشي
يجب أن تركّز على: السيطرة على الألم، النوم، الذهاب إلى دورة المياه بنفسك، تحريك الورك، والوقوف والمشي أكثر قليلًا كل يوم.
قبل موعد فحص الجرح في الأسبوع الثاني يجب أن تكون:
- تمشي إلى دورة المياه وتستخدمها بنفسك. توقّف عن استخدام المبولة أو الكرسي المتنقل بمجرد أن تستطيع الوقوف بالمشّاية — عادةً خلال الأيام الأولى. البقاء على المبولة يؤخر كل شيء آخر.
- تُدخل ساقك إلى السرير وتُخرجها منه بنفسك (يساعدك في البداية حزام أو رباط حول القدم)
- تجلس على الكرسي المرتفع والسرير والمرحاض المرتفع وتنهض منها دون مساعدة
بنهاية الأسبوع الثالث يجب أن تستطيع:
- المشي داخل البيت بالمشّاية أو العكازين
- صعود الدرج ونزوله بأمان مع أداتك: صعودًا بالساق السليمة أولًا، ونزولًا بالساق المعالَجة أولًا
التمارين: تحريك الكاحلين كل ساعة؛ تمارين الورك ٣–١٦. امشِ عدة مرات قصيرة في اليوم بدلًا من مرة واحدة طويلة.
الاستحمام: يمكنك الاستحمام تحت الدش من الأسبوع الثاني، بعد فحص الجرح. لا حوض ولا مسبح ولا بحر حتى الأسبوع الرابع، وبعد أن يلتئم الجرح تمامًا.
المواعيد: فحص الجرح بعد أسبوعين. في هذه الزيارة تُزال الضمادة ويُفحص الجرح. وينبغي أن يكون الجرح مغلقاً وجافاً من غير رشح. إذا كان الجرح قد أُغلق بدبابيس (ستيبلر) أو بغرز تحتاج إلى إزالة، فتُزال في هذه الزيارة؛ يستغرق ذلك دقائق قليلة ولا يحتاج إلى تخدير. وإذا كان جرحك ما يزال يرشح في أي وقت بعد الأسبوعين، فلا تنتظر الموعد التالي — تواصل مع العيادة واحجز موعداً في أقرب وقت ممكن.
الأسابيع ٣–٦ · مدى الحركة والتحكم
يجب أن تركّز على: ينبغي أن يقلّ ألم الورك وتسهل حركته. اعمل على المرونة والقوة والتحكم في الساق.
بنهاية الأسبوع السادس يجب أن تستطيع:
- ثني الورك إلى ٩٠ درجة، وتحريك الساق إلى الجانب، ومدّها بالكامل
- المشي عدة مئات من الأمتار بالعصا في اليد المقابلة
- التبديل على دراجة ثابتة عمودية والمقعد مرفوع، في العيادة أو النادي
التمارين: أضف تمارين الورك ١٧–٢٤. التمرين ١٩ يبدأ من الأسبوع السادس.
القيادة: الورك الأيسر وسيارة أوتوماتيك — متى ما ارتحت في مقعد السائق وتوقفت عن المسكّنات القوية. الورك الأيمن — ليس قبل ٦ أسابيع. وفي الحالتين استشر جرّاحك أولًا.
المواعيد: مراجعة الجرّاح بعد ٦ أسابيع. هذه الزيارة وفحص الجرح في الأسبوع الثاني هما أهم زيارتين؛ لا تفوّتهما.
الأسابيع ٦–٩ · العودة إلى الحياة اليومية
تنتهي الاحتياطات في الأسبوع السادسيجب أن تركّز على: العودة إلى الأنشطة الطبيعية، وتمارين أصعب بالاتفاق مع أخصائي العلاج الطبيعي.
بنهاية الأسبوع التاسع يجب أن تستطيع:
- المشي مسافات أطول بالعصا دون عرج
- النهوض من كرسيك المرتفع والجلوس عليه دون الاستعانة بيديك
- التوازن لحظات على الساق المعالَجة
- صعود الدرج قدمًا على كل درجة، مع الدرابزين أو العصا
- قضاء نزهة أطول — جولة في مجمّع تجاري، زيارة عائلية — دون أن تدفع ثمنها في اليوم التالي
التمارين: أضف ٢٥–٣١ وتدرّج التمرين ١٤ب.
مسموح الآن: العودة التدريجية إلى الجلوس على الأرض وأوضاع الصلاة كاملة، والنوم على الجانب دون وسادة، والكنب المنخفض، والانحناء للبس الجوارب والحذاء بنفسك.
الأسابيع ٩–١٢ · الثقة والقوة
يجب أن تركّز على: الثقة في الحياة اليومية، وبناء القوة والتوازن للمدى الطويل.
بنهاية الأسبوع الثاني عشر يجب أن تستطيع:
- المشي دون عرج ودون عصا
- النهوض من كرسي ومرحاض بارتفاع عادي دون الاستعانة بيديك
- صعود الدرج قدمًا على كل درجة مع القليل من الدرابزين أو دونه
- الانضمام إلى نادٍ رياضي أو مجموعة مشي
التمارين: أضف ٣٢–٣٧ والتدرّجات المتأخرة للتمارين ١٤ و٢٢ و٢٣ و٢٨ و٣١.
المواعيد: مراجعة الجرّاح بعد ٣ أشهر.
من ٣ أشهر إلى سنة · اللياقة والرياضة
يجب أن تركّز على: النشاط المنتظم فهو يحافظ على صحة المفصل الجديد ويطيل عمره. تحتاج الساق المعالَجة إلى سنة أو أكثر لتستعيد قوتها كاملة، فواصل ولا تيأس.
كيف تستمر في التقدم
- القوة: ٨–١٥ تكرارًا؛ ينبغي أن تشعر بتعب العضلة عند التكرار الأخير. إن لم تستطع ٨، خفّف التمرين. وعندما تستطيع ١٥ في ثلاث جلسات متتالية، صعّبه: أضف ١–١٫٥ كغم أو وسّع المدى. يومان في الأسبوع يحفظان القوة، وثلاثة يبنونها. استرح في اليوم التالي، ولا تتجاوز أربعة أيام قوة في الأسبوع أبدًا.
- التوازن: ٦–٨ تكرارات، ٣ مرات في اليوم، ودائمًا مع شيء تمسك به في متناول يدك.
- المرونة: ثبّت الوضعية ٣٠ ثانية، ٣–٤ مرات، ثلاثة أيام في الأسبوع على الأقل، والعضلات دافئة.
العودة إلى العمل: عادةً بعد مراجعة الأشهر الثلاثة. إذا كان عملك مكتبيًا وتريد العودة أبكر، اسأل جرّاحك في مراجعة الأسبوع السادس.
المواعيد: مراجعة الجرّاح بعد ٦ أشهر وبعد سنة. واحجز موعدًا في أي وقت بينهما إذا أقلقك شيء.
الرياضة والنشاط
اختر أنشطة قليلة الصدمات وتعرفها من قبل. تعلّم رياضة جديدة على ورك جديد يعني سقطات أكثر.

للمقارنة بين مدى إجهاد كل نشاط للمفصل: الدراجة تحمّل الورك نحو ١٫٤ ضعف وزن الجسم، والمشي السريع نحو ٤٫٧ أضعاف، والهرولة نحو ٦ أضعاف.
أمور طبيعية في هذه المرحلة
- تورّم في الفخذ والساق، وخاصة في القدم والكاحل في الجهة المعالَجة، يزداد في نهاية اليوم. هذا أكثر سؤال يصلنا، والجواب أنه يزول خلال أسابيع إلى أشهر. ارفع ساقيك عند الراحة، وحرّك كاحليك، وامشِ مرات قصيرة بدلًا من مرة طويلة، وتجنّب الحرّ. وحرّك أصابع قدميك كثيراً: هذا يشغّل عضلات القدم ويدفع السائل إلى أعلى الساق.
- خدر بجانب الجرح؛ وغالبًا يبقى.
- طقطقة أو إحساس بأن المفصل "يستقر" في الأسابيع الأولى.
- اضطراب النوم حتى ٣ أشهر.
- زيادة مؤقتة في الألم والتورّم عند بدء تمرين جديد أو العودة إلى العمل. استخدم الثلج أو الكمادات الباردة، وخذ راحة، وعُد إلى النشاط تدريجياً.
انتفخت ساقك كاملةً أو أصبحت دافئة أو ازداد ألمها؛ أو احمرّ الجرح أو انفتح أو خرج منه سائل؛ أو أصبت بحمّى؛ أو اشتدّ الألم فجأة بشدة؛ أو بدت الساق المعالَجة فجأة أقصر أو انقلبت القدم إلى الخارج ولم تستطع تحريكها — فقد يكون ذلك خلعًا، توجّه إلى الطوارئ.
توجّه إلى الطوارئ فورًا عند ألم في الصدر أو ضيق مفاجئ في التنفس.
